  • Negative Results of a Drug and Alcohol Screen.
Once the Support Services Department receives satisfactory results from the above pre-employment screening process, the Support Services Department will notify the candidate to confirm the initial employment offer and start date. I also understand that I am required to abide by all policies and procedures of TAC ENTERPRISES and that this is an application and is not intended to be a contract of employment.

Disclosure and Release Criminal Record Information

In connection with my application for employment with TAC Enterprises, I understand that consumer reports which may contain public record information may be requested from the Bureau of Criminal Identification and Investigation (BCII), Federal Bureau of Investigation (FBI), and local or out of state sheriffs’ offices. These reports may include the following types of information: names and dates of previous employers, reason for termination of employment, work experience, accidents, etc. I further understand that such reports may contain public record information concerning my driving record, workers’ compensation claims, credit, bankruptcy proceedings, criminal records, etc. from federal, state and other agencies which maintain such records.

A criminal records check is required to be conducted and satisfactorily completed in accordance with Section 109.572 of the Ohio Revised Code if the applicant comes under final consideration for appointment or employment as a precondition of employment in a position. If the applicant comes under final consideration for appointment or employment the cost of the BCII is paid for by TAC Enterprises.

I authorize, without reservation, TAC Enterprises to contact the abovementioned agencies and any party or agency contacted by those agencies to furnish the abovementioned information.

I hereby give the Sheriff of Clark County, Ohio or his authorized Deputy, permission to release any records that I may have to TAC ENTERPRISES Support Service Department. I hereby release the Sheriff of Clark County, Ohio and/or his authorized deputy from any liability arising from information given, as guaranteed under the Privacy Act.

I hereby authorize procurement of consumer report(s). If hired this authorization shall remain on file and shall serve as ongoing authorization for TAC Enterprises to procure consumer reports at any time during my employment period.

Affidavit of OH Residency and Felony Charges Convictions

The applicant states that he/she has been a resident of Ohio for the five year period preceding this application. The applicant agrees to provide proof upon request to TAC that he/she has been a resident for the five year period preceding this application.he/she has not been a resident of OHIO for the five year period preceding this application (includes out-of-state university and military experience).
